Direct Primary Care Membership

 Adult  $100/month 


Children  $25/month


Two Adult household  $175/month


 Family: $175/month for two adults, plus $25/month for each of the first three children. Additional children are included.


Family maximum: $250/month.



Membership includes:

  • Longer, unrushed appointments
  • Priority scheduling
  • Direct access to your provider, including after-hours cell access for appropriate medical concerns
  • Preventive, acute, and chronic primary care
  • Routine in-office point-of-care testing
  • Care coordination and referrals
  • Access to available cash-pay pricing for labs, imaging, and local pharmacy services

* Children may enroll independently at the $25/month rate; a parent does not need to be a DPC member.

* Home visits may be available to members for an additional $100, depending on location and availability.

Cash-pay office visits are $150. No membership or insurance billing is required. Routine in-office point-of-care testing is $25 per test for non-members.


A detailed receipt or superbill is available upon request for patients who wish to submit their visit to their insurance for possible out-of-network reimbursement. Insurance reimbursement is determined by your individual plan and is not guaranteed.

Minor Procedures & Wound Care

 Suturing, incision and drainage, wound care, and other minor procedures may be available when clinically appropriate. Pricing is discussed with you before treatment begins, whenever possible. 

Membership Is Not Health Insurance

  

Story Health’s Direct Primary Care membership is not health insurance and is not intended to replace health insurance coverage. Membership covers only the primary care services specifically included in your membership agreement.


Patients are encouraged to maintain health insurance or other coverage for services outside Story Health, including emergency care, hospitalization, specialist care, imaging, outside laboratory testing, prescriptions, and other services not included in the membership. BioTE® hormone pellet therapy and other separately priced services are not included.

Forms and Privacy

 A copy of our Notice of Privacy Practices is available upon request and will be provided through Elation Patient Passport. 

Cancellation Policy

Please provide at least 24 hours’ notice to cancel or reschedule. Appointments cancelled with less than 24 hours’ notice, or missed without notice, may incur a $50 fee. Fees may be waived for emergencies or circumstances beyond the patient’s control. 

Payment Policy

A payment method is securely saved when booking but is not charged at that time. Cash-pay visit fees and other authorized charges are due at the time of service. DPC membership charges begin only after enrollment is accepted.
